The way I understand the whole handball penalty rule is that its all about intent, so if a defender moves his hand to ball or if the defender has his arms out making himself a bigger and the ball hits his hand then its a pen because he has intentionally put his arms in a position to possibly block the ball. Neither of these apply to what happened with Insua the other night, it was never a penalty, unlike the challenge on Kuyt which clearly should have been a pen, obviously not for hand ball though
