Broadband Question(s)

In my experience when you get 'broadband' (aka 'ADSL' or 'DSL') they give you a 'router' (possibly wireless, possibly not).

 

But what is a 'broadband modem' such as the one pictured here?

Is this really a 'modem'? Does it actually 'dial' something at the other end?

 

How do you set such a thing up? The normal 'log onto router and set the configuration' approach doesn't seem to apply.
