Specialist Travel Insurance

Looking for a few pointers about specialist travel insurance - cancer patients in particular, for older people. Anyone got any experience of booking this sort of thing? The usual travel companies massively inflate their premiums where illness is concerned or won't offer cover so there must be some specialist companies out there?
